Akalabeth is the precursor of the Ultima series - actually it is often referred to as 'Ultima 0'. This very first edition was created by Richard Garriott based on earlier, text-based mini games (D&D1 - D&D28). After some substantial investments in ziploc bags, disks and xerox copies he self-published it in a local computer store chain where he took a semester job. All in all he made about 20 copies this way of which 12 were sold. The games' ziploc bag contained a selfmade stapled instructions booklet, a bigger cover sheet and the disk with simple photo-copied labels. The original 12 are supposed to be hand numbered on a small green sticker.
